The driving factor behind Tony Chase's internet following is his poor mental health and delusions, Tony suffering from a possible lifetime of untreated schizophrenia (or another/more delusional disorders). He does not appear to be under the care of a psychiatrist, and it is unknown if he has ever seen one.
Tony is definitely schizophrenic. It's not just his extreme delusions that lead to the conclusions, but him demonstrably suffering from hallucinations. An example of this are his claims of the Satellite Man "zapping him," which are most likely sensory hallucinations (although it is possible he is suffering from some sort of medical disorder, such as fibromyalgia, but these are harder to prove and Tony doesn't display the other symptoms of most of these disorders).
