Tony is definitely schizophrenic. It's not just his extreme delusions that lead to the conclusions, but him demonstrably suffering from hallucinations. An example of this are his claims of the Satellite Man "zapping him," which are most likely sensory hallucinations (''although'' it is possible he is suffering from some sort of medical disorder, such as fibromyalgia, but these are harder to prove and Tony doesn't display the other symptoms of most of these disorders).
